I, the undersigned participant, hereby acknowledge and agree to the terms and conditions outlined in this Martial Arts Championship and Photo/Video Release Form ("Form") in consideration for my participation in the Jinheng Li' Wushu Holiday championship event ("Event") organized by [world martial arts academy].

 

1. Assumption of Risk:

I understand that participating in the Event involves inherent risks, including but not limited to physical injury, property damage, or even death. I voluntarily assume all such risks and agree to release, indemnify, and hold harmless [world martial arts academy/Jinheng Li/tournament's director/judges/volunteers ], its affiliates, sponsors, and their respective officers, directors, employees, and agents from any and all claims, liabilities, or expenses arising out of or in connection with my participation in the Event.

 

2. Consent to Medical Treatment:

In the event of any injury or illness during the Event, I authorize [Event Organizer] and its representatives to obtain medical treatment on my behalf, including but not limited to emergency medical services, hospitalization, or surgery, if deemed necessary. I understand that I am responsible for any associated medical expenses.

 

3. Photo/Video Release:

I grant [Event Organizer] and its authorized representatives the irrevocable right and permission to photograph, record, and use my name, likeness, voice, and performance in any media format, including but not limited to photographs, videos, audio recordings, and social media platforms, for promotional, advertising, or educational purposes related to the Event. I understand that I will not receive any compensation for such use.

 

4. Intellectual Property Rights:

I acknowledge that all intellectual property rights, including copyrights, related to any photographs, videos, or other media captured during the Event shall belong solely to [Event Organizer]. I agree not to reproduce, distribute, or use any such media without the express written consent of [Event Organizer].

 

5. Confidentiality:

I understand that certain aspects of the Event, including but not limited to training techniques, strategies, and event organization, may be considered confidential. I agree not to disclose or share any confidential information obtained during the Event without the prior written consent of [Event Organizer].

 

6. Governing Law and Jurisdiction:

This Form shall be governed by and construed in accordance with the laws of [State/Country]. Any disputes arising out of or in connection with this Form shall be subject to the exclusive jurisdiction of the courts located in [City/County/State].
